Eligibility First

Is WISe the right level of support?

WISe may be helpful when a child or teen needs more support than regular outpatient counseling can provide.

  • Your child is between ages 5–21.
  • Your child has Apple Health / Medicaid, or you need help confirming coverage.
  • Behavioral health needs are affecting home, school, community, or safety.
  • There may be repeated crises, school problems, safety worries, or several people already involved in your child’s care.
  • Your family can take part in regular meetings and services with the care team.

If you are unsure whether WISe is the right program, you can still start with the request form or call Three Rivers Therapy.

What WISe Includes

Support built around your child and family

WISe is flexible, and team-based. The plan is built around your family’s strengths, goals, and daily life.

Care coordination

Help bringing providers, schools, family supports, and community partners into one shared plan.

Therapy and peer support

Support may include therapy, support for your child, and support for caregivers from people who understand what families go through.

Crisis planning

The team helps your family plan for difficult moments and receive support at home, at school, and in the community.

Understanding WISe

WISe is not standard weekly counseling

WISe is a higher level of support for families who need more than a weekly therapy appointment.

WISe may help when

  • Your child's needs feel hard to manage alone
  • Support is needed in more than one setting
  • Weekly counseling has not been enough
  • Your family needs a team helping with one plan

Another service may help when

  • You are looking for regular outpatient therapy
  • The main need is medication management
  • You need testing or a one-time diagnosis
  • Your child does not have Apple Health / Medicaid
