Eligibility First
Could FCS Be a Fit?
FCS is a targeted Apple Health benefit for people facing significant barriers to stable housing or employment.
Eligibility considers Apple Health coverage, age, a needs assessment, and qualifying medical or behavioral health risk factors.
You do not need to determine eligibility alone. Contact Three Rivers Therapy and share what kind of support you need.
FCS May Be Worth Asking About If You:
- Are enrolled in or eligible for Apple Health
- Face significant barriers to finding or keeping housing or employment
- May meet applicable age and risk-factor requirements
- Are willing to complete an eligibility and needs review
How FCS Can Help
Practical Support Built Around Your Goals
FCS includes two targeted benefits. The right starting point depends on your goals and eligibility.
Supportive Housing
Get help finding and keeping community housing.
- Rental application support
- Help identifying housing resources
- Connections to potential assistance programs
- Ongoing support for housing stability
Supported Employment
Get help preparing for and pursuing work.
- Résumé development
- Job searches and applications
- Employment preparation and coaching
- Support with barriers to keeping a job
Understanding FCS
Support and Guidance not Direct Payments
The value of FCS is hands-on help: coaching, advocacy, information and referrals, coordination, and ongoing support.
What FCS Does Not Pay For
FCS itself does not pay rent, rental subsidies, room and board, wages, or wage supplements.
Some eligible participants may qualify for separate housing-assistance programs. A case manager may help explore available resources, but assistance is not guaranteed.
