Outpatient Counseling

Experience personalized outpatient counseling tailored to your needs. Our compassionate counselors provide support and guidance to help you navigate life’s challenges. Take the first step toward positive change and emotional well-being. We have counselors dedicated to children, young adults, individuals, family and couples. 

WISe

Wraparound with Intensive Services (WISe) is an approach to helping Medicaid-eligible children, youth, and their families with intensive behavioral health needs. Services are available in home and in community settings. WISe is available for individuals aged 0-21, offering a tailored approach to address a wide range of challenges, including mental health concerns and behavioral issues. Our dedicated team, including bilingual providers, works collaboratively with clients and their support networks to create personalized plans that promote stability, well-being, and long-term success.

Peer Hub

Our Peer Hub is a safe and friendly community forum where individuals can develop the life skills they need to succeed. Our Peer Hub woks to remove the barriers or limitations that individuals often experience due to mental health distress or disorders. 

FCS

Foundational Community Supports (FCS) is a program which offers benefits for supportive housing and employment services to Apple Health-eligible participants with a Substance Abuse or Behavioral Health diagnosis. Our case managers can help with everything from rental applications and financial assistance for homeless individuals to building resumes and applying for jobs with those looking to re-gain employment. 

Peer Respite 

Three Rivers Therapy Peer Respite Center is a home like non-clinical alternative to hospital psychiatric ER’s and inpatient units. Guests can stay up to 7 days with flexibility to come and go for appointments, jobs and other essential needs. Our respite is 100% operated by peers who have their own lived experience. Our Peer Respite serves Medicaid insured individuals who are experiencing mental health crisis but do not meet criteria for detainment. Individuals must be 18 and older and be entering respite voluntarily. 

OHY 

The OHY (Office of Homeless Youth) Street Outreach Program, a proactive initiative dedicated to reaching out to vulnerable youth aged 12-24 who are experiencing homelessness or at risk of homelessness. Our street outreach teams tirelessly engage with individuals on the streets, offering vital support, resources, and connections to services. Through our outreach efforts, we strive to provide immediate assistance while also empowering young individuals to access pathways to stability and brighter futures. 

Medication Management

Mary Kimani, our Advanced Registered Nurse Practitioner is now accepting new patients that are currently established in services at Three Rivers Therapy. Mary has experience working with individuals experiencing both medical and psychiatric illnesses. She has worked in different settings including acute care, outpatient, long-term care, and corrections among others. Mary offers evaluation, diagnosis, and medication treatment for psychiatric mental illnesses. Mary works closely with our team of behavioral health counselors to provide care for you. She is now seeing clients 8 years and older.
